Template For ObituaryHaving a template for obituary publication is important when writing an obituary to place in a newspaper or an online memorial.

Someone you loved has passed away and it is an emotional time. Thinking of the right words to place in an obituary can be difficult. Also, there are certain guidelines that you need to follow when writing an obit and this template will make sure you include the required information.

You may also want to ask the funeral home and other family members for assistance. Start by gathering all of the necessary facts and then use the template for obituary below to write a beautiful obit for your loved one.

INSTRUCTIONS: Replace all of the items in RED below with the appropriate information.

[NAME] is predeceased by [HIS OR HER RELATION], [NAME] (list all family who are predeceased).

[NAME] is lovingly remembered by [HIS OR HER] [RELATION], [NAME] of [CITY] (list children; grandchildren; great-grandchildren; brothers; sisters; nieces and nephews. Including their home city is optional).

[NAME] was born in [CITY], [STATE] in [DATE]. [HE OR SHE] graduated from [HIGH SCHOOL NAME] in [GRAD YEAR] and went on to earn a [DEGREE/CERTIFICATE] from [SCHOOL NAME] in [YEAR]. [HE OR SHE] worked as [JOB AND POSITION].

EXAMPLE OBITUARY

Andrew “Andy” Eric Phillips passed away at his home in Seattle on Friday, April 6, 2012 at the age of 72.

Andrew is predeceased by his parents, Jack and Marilyn Phillips, and his brother, Benjamin Phillips.

Andrew is survived by his loving wife Jennifer of 47 years. They were married in Hawaii on June 20, 1970.

Andrew is lovingly remembered by his wife Jennifer of Seattle; brother Matthew (Wendy) Phillips of Seattle; sister Amanda (John) Walters; sons Bill and James Phillips; and loving nieces, nephews and other relatives.

Andrew was born in Denver, Colorado in 1940. He graduated from Denver High School in 1957 and went on to earn a Degree in Business from Colorado University in 1962. He worked as an Operations Manager at the Seattle Post.

Andrew enjoyed sports, especially football, as well as movies, reading, volunteering and spending time with his family.

The visitation will be held at Jones and Jones Funeral Home, 555 Tree Lane, Seattle, WA on April 8, 2012 at 10:00 am. Prayers will be offered from St. David’s United Church, 943 West 45th Street, Seattle, WA on April 8 at 3:00 pm.

The funeral service will be held at Jones and Jones Funeral Home, 555 Tree Lane, Seattle, WA on April 9, 2012 at 10:00 am with Rev. Mike Jones officiating. Interment to follow at Tree View Cemetery, 490 Mountain Lane, Seattle, WA at 3:00 pm. A wake will take place at Jennifer Smith’s House on April 9, 2012 at 7:00 pm.
